It is early to say what the actual cause was, but it certainly appears to be operator error. The tracks and signals were all operating correctly. The first train had just moved forward after correctly stopping at a signal just before entering the station. The second train collided with it at approximately 37 MPH, just under to 40 MPH speed limit for that section, but certainly above what you would expect entering the station. Riders reported that the driver of the second train was chatting with someone on her cell phone when the accident occurred.

These agencies always take forever to come to a decision. When they do you know it is most likely the correct one. Enough time has passed for the issue to be remembered except by those involved. That takes all the pressure off of a "correct" version.
